Application notes - please read before beginning an application:

Please read About Registration and Pathways to registration before you begin your application. You must have joined a New Zealand professional society, before becoming eligible to register. Do not begin the registration process if you have not yet joined the relevant professional society for your profession.
It will help to have details of your employment history and educational record (e.g your CV) on-hand to complete the online form.
Payment can be made at the end of the form using a credit card via Paypal, or separately using online banking.
If you are a student residing outside of New Zealand, you cannot apply for registration in New Zealand until you are qualified.
If you are applying from outside of New Zealand, please make payment using a credit card via Paypal to ensure the correct fee is paid. Select the "Registration - non resident" option below.
If you were previously registered with CPRB, but cannot see your name on the Register, please email the Registrar.

How to register - 3 steps:

1. DOCUMENTS: Download and print this form, complete and sign - return with requested documents to CPRB via post or scan & email.

2. ONLINE REGISTER: Select Registration Level below, and follow the on screen instructions to create your Register profile. Complete by clicking on 'CONFIRM'

3. PAYMENT: Make payment - instructions for payment by credit card or internet banking will appear at the end of the online form.

All 3 steps must be performed to complete your application.

(Note: the category "Physiology Technician" applies within the Cardiac scope of Practice only. To determine whether to apply for physiologist or technician registration, please refer to the Cardiac Competencies outline, via this link Registration Competencies )

Certification of Qualifications:

CPRB does not recognise any qualification unless the applicant submits a certified copy - this means a copy is made of the original document, and presented to a Justice of the Peace, Solicitor, Public Notary or similar with the original document. The certifier stamps the copy with their official stamp, and signs the copy, attesting that it is a true and unadulterated copy of the original document.

Photo ID:

You are required to provide a good quality colour copy of photo ID, e.g. a scan of your passport or drivers license. The photograph portion only will be uploaded to the register and is able to be seen by the public.
